Non-communicable diseases (NCDs), particularly diabetes mellitus (DM), have become a major public health concern worldwide. According to the International Diabetes Federation, more than 537 million people were living with diabetes in 2021, and the number is expected to continue rising in the coming years. This increase is especially evident in middle-income countries such as Indonesia, where rapid lifestyle changes, unhealthy dietary habits, and limited preventive health practices contribute to the growing burden of the disease. At th e local level, Sukamulya Village in Cihaurbeuti District, Ciamis Regency shows several risk factors associated with diabetes mellitus. These include high consumption of sugary and fatty foods, low levels of physical activity, and limited awareness of regular blood glucose monitoring among community members. Such conditions highlight the need for effective community-based preventive interventions. This study aims to identify the determinants of diabetes and evaluate the effectiveness of the SEHATI Program (Healthy Spinach Cultivation and Physical Activity for Diabetes) as a promotive and preventive effort in rural communities. A quantitative approach with a cross-sectional design was applied to 234 respondents selected through stratified random sampling. Data were collected through questionnaires and direct observation. The program significantly improved knowledge and encouraged healthier lifestyles among participants overall.
